Johnny Dale Smith, 55, of Jamestown, passed away Thursday, November 28th, at his residence.
Johnny was born in Cincinnati, Ohio on August 31, 1969, a son of the late JoAnn (Mills) Warriner and Evert Smith.

Four Sisters, Karen Frazier, Cathy Johnson, of Jamestown, KY, Treva Smith, of Russell Springs, KY, Teri Ferrari, of Cincinnati, OH.
One Brother, David Smith, of Russell Springs, KY.
Funeral Service will be held 1:00 PM on Tuesday, December 3rd at H.E. Pruitt Memory Chapel in Jamestown, KY with Bro. Bobby Dunbar officiating.
Visitation will be from 12:00 to 1:00 PM on Tuesday December 3rd at H.E. Pruitt Memory Chapel in Jamestown, KY Interment at Sunshine Cemetery.
